Print and complete this form in the presence of a notary. One form per student is required annually even if you do not plan on utilizing AfterCare.

CCS Aftercare $155/mo (3+days/wk regular daily attendance) – ALL STUDENTS

CCS Aftercare $10/day (1-2 days regular daily attendance/wk) – ALL STUDENTS
Emergency Drop-IN
CCS Aftercare $30/day drop-in attendance for all programs. – ALL STUDENTS
AfterCare Payments
Payments for AfterCare will be added to your monthly school tuition. – ALL STUDENTS
